Once you have received your contract, please download and return the new staff packet below by the date listed on your contract. The new staff packet includes:

• Health Form
• Verification Form
• 2010 W-4 Federal Tax Withholding Form
• 2010 I-9 Employment Eligibility Verification Form
• Background Check Authorization

Please include a copy of the following pieces of information:

• Picture for staff orientation handbook (You may email to: doug@campkiowa.com)

• Driver's license

• Student ID (if you're a student)

• Social Security Card

• CPR & First Aid Certification Card (required)

• Lifeguard Certification Card (If completed.  Certification not required)
